Finish your intake to finalize your session

Submit both files at least 24 hours before your appointment or the system will auto-cancel your call and email a reschedule link.

This page contains two clear steps:

1) Download the intake form templates.

Then after you complete them,

2) Upload completed files below.

Step 1 — Download your templates

If you need more time to fill the templates out, a link has been sent to your email so that you can return to this page.

Open in Excel, complete fully, and save as:

Lastname_Firstname_MC.xlsx and

Lastname_Firstname_LMH.xlsx

For example, Billy Joe would name his files:

Joe_Billy_MC.xlsx

Joe_Billy_LMH.xlsx

Step 2 — Upload your completed files below

Accepted: .xlsx only (Excel). Max 20MB each. Submit ≥ 24 hours before your appointment.

Name files exactly:

  • Lastname_Firstname_MC.xlsx

  • Lastname_Firstname_LMH.xlsx

Submit at least 24 hours before your appointment or your session will auto-cancel.

If you uploaded the wrong file, reply to your confirmation email immediately.
